Hawaii Test Accommodation Verification Request Form 2022-2023 (Online Version)

The SY 2022-23 Hawaii Test Accommodation Verification Request Form is available electronically for all general assessments. General assessments in Hawaii include Smarter Balanced and HSA Science (NGSS) assessments, and the Biology, Algebra 1, and Algebra 2 End-Of-Course (EOC) exams. When using the form, remember to substantiate accommodation requests with evidence found in the student's IEP or 504 records. Only one form is needed to request all the test accommodations for a student.
