Hawaii Test Accommodation Verification Request Form 2021-2022 (Online Version)
Jan 1, 0001The 2021-22 Hawaii Test Accommodation Verification Request Form is available electronically for all general assessments. General assessments in Hawaii include Smarter Balanced, HSA Science (NGSS), and KA'EO assessments, and the Biology, Algebra 1, and Algebra 2 End-Of-Course (EOC) exams. The Hawaii State Test Accommodation Verification Request Form should not be submitted for the HSA-Alt, WIDA, NAEP, or ACT tests. When using the form, remember to substantiate accommodation requests with evidence found in the student's IEP or 504 records. Only one form is needed to request all the test accommodations for a student.
